Insurance payor ( Party Billing)
MultiGen Diagnostics Inc accepts third-party billing for customers. We attempt to bill most of the insurance carriers including Medicare; however, it is the customer’s responsibility to clarify specific participation information by contacting their member services. For Medicare, please be sure to include the Medicare HIC number, which includes 9-digit number and alpha prefix.
Note: Patients are responsible for yearly deductibles and any balances not covered by their insurance company.
Private pay (Patient Billing)
Patients may choose to billed directly for laboratory services. When direct billing is requested, the following information must be supplied in the patient information area of the test requisition form:
1. Name of responsible party
2. Current address and phone number for patient and responsible party (if other than self)
3. Social Security Number (SSN)
Note: If a patient does not have insurance coverage, the patient will be billed directly.
